A Complete Referral Helps Us Start Sooner

- Client details, address and type of accommodation
- Medical conditions and other services in place
- Nursing services required, such as wound care, personal care or medication management

Please do not send the referral form to DVA. It goes directly to a DVA approved Community Nursing provider.
Common Referral Questions
Which form should I use?
Use our online referral form, or the DVA Community Nursing Referral (D9389), available on the DVA website. Send it directly to us, not to DVA.
How long is a referral valid?
Referrals from a GP or nurse practitioner are valid for 12 months. Referrals from a hospital are valid for 6 weeks after discharge, after which the GP needs to provide an updated referral.
Do you need a Medication Authority?
Yes, if medication management is requested. Please attach a signed Medication Authority or order from the treating doctor.
How quickly will you respond?
We acknowledge referrals within one business day and contact the client to arrange their first assessment.
Will you keep me informed?
Yes. The client's GP keeps ongoing clinical oversight, and we send progress updates and let you know about any change in condition.
