About this policy

Community Care Provider is committed to protecting the privacy of our clients, their families, referrers and staff. This policy explains how we collect, use, store and disclose personal and health information in line with the Privacy Act 1988 (Cth) and the Australian Privacy Principles.

Information we collect

To provide safe nursing care we collect information such as name, address, date of birth, contact details, DVA file number and card type, medical history, current medications, clinical notes, and details of other services received. We also collect referrer details, including name, role, practice and provider number.

How we collect it

Most information comes from the referral provided by a GP, hospital practitioner or nurse practitioner, from the client and their family, and from our nurses during assessments and visits. With consent, we may also receive information from other health providers involved in the client's care.

How we use it

We use personal information to assess eligibility, plan and deliver nursing care, communicate with the client's GP and treating team, claim payment from the Department of Veterans' Affairs, meet our legal and accreditation obligations, and improve the quality and safety of our services.

Who we share it with

We only disclose information for the purpose it was collected, or where the law allows. This includes the client's GP and treating health professionals, DVA, and other providers involved in the client's care. We do not sell personal information and we do not send it overseas.

How we store and protect it

Records are kept in secure electronic systems with restricted, password-protected access. Paper records are stored securely and destroyed confidentially. Health records are retained for the period required by South Australian law.

Access and correction

You can ask to see the information we hold about you, or ask us to correct it. Please contact us in writing. We will respond within 30 days and may need to confirm your identity first.

Website forms

Information submitted through our Refer a Client and Contact forms is sent to a secure inbox and used only to respond to your referral or enquiry.
