Who Can Receive Our Services

To be eligible for our community nursing services, clients must meet one of the following criteria.

A Veteran Gold Card enables a client to receive health care and related services to meet all of their assessed clinical nursing and/or personal care needs.

Hold a Veteran White Card

With eligibility for medical treatment related to accepted service-connected injuries or conditions, or under Non-Liability Health Care. We contact DVA to confirm eligibility before care begins.

No cost to you. If you are eligible, DVA funds the services we deliver. War widows and widowers who hold a Veteran Gold Card are also eligible.

Questions Families Often Ask

Call us on 0433 000 586 to speak with a nurse.

Is there a cost to me?

No. If you are eligible, DVA funds the Community Nursing services we deliver, so there is no cost to you.

Do I need a referral?

Yes. You need a written referral from your GP, a hospital doctor or discharge planner, or a nurse practitioner. They send it directly to us.

How often will a nurse visit?

Visits are based on your assessed clinical need. After your first assessment, your nurse sets out how often they will visit in your written care plan.

Can my family be present?

Of course. Family members and carers are welcome at every visit, and we are happy to include them in planning your care.

Are war widows and widowers eligible?

Yes. War widows and widowers who hold a Veteran Gold Card can receive Community Nursing services for their assessed clinical need.

What if I move into residential aged care?

Community Nursing is only available to people living in their own home or an Independent Living Unit. If you move into a Residential Aged Care Facility, the facility provides your nursing care.
